PARTICIPANT WAIVER & RELEASE AGREEMENT

This form must be completed by the participant or the participant’s parent/legal guardian before participating in PUCK—IT programs, camps, clinics, tournaments, or development sessions.

Assumption of Risk

I acknowledge that participation in hockey, skating, training, camps, clinics, tournaments, and athletic activities involves inherent risks, including injury, illness, contact, falls, collisions, equipment-related injuries, and other risks associated with sport participation.

Release of Liability

I release and hold harmless PUCK—IT Development, Pro Shops by PUCK—IT, its owners, staff, instructors, contractors, volunteers, affiliates, and facility partners from claims, demands, damages, actions, or liability arising from participation, except where prohibited by law.

Medical Treatment Authorization

In the event of injury or illness, I authorize PUCK—IT staff or representatives to seek emergency medical assistance for the participant if I cannot be reached.

Parent/Guardian Consent

I confirm that I am the participant or the parent/legal guardian of the participant and have authority to provide consent for participation in PUCK—IT programs and activities.

Code of Conduct

I agree that the participant, parent/guardian, and spectators will act respectfully toward PUCK—IT staff, instructors, officials, facilities, and other participants. I understand that unsafe, abusive, threatening, or disruptive behaviour may result in removal without refund.

Refund Policy Acknowledgement

I acknowledge that I have read and agree to the PUCK—IT Refund & Cancellation Policy.
